Duplicate servers in the lists; Forum session issues

Two quick updates:

- It should now be next to impossible to add a duplicate server to the list or to edit the details of a server to match the details of an existing server. If you see any duplicates in a list, please post them here!

- Last night there was an issue with the forums where everyone was getting a session verification error which affected ability to post or even to logout. This was caused by an upgrade I was performing and was fixed later.
